Domestic pigs were randomized to control, RVH, and RVH treated with Valsartan (320 mg/day; RVH+Valsartan) or triple-therapy (Reserpine+hydralazine+hydrochlorothiazide, RVH+TT) for 4 wks post 6-wks of RVH (n=7 each). Eligible patients were 18 years of age or older and had normal sinus rhythm, no contraindication to warfarin therapy, and an LVEF of 35% or less as assessed by quantitative echocardiography (or a wall-motion index of ≤1.2) or as assessed by radionuclide or contrast ventriculography within 3 months before randomization. Patients who had a clear indication for warfarin or aspirin were not eligible. Patients in any New York Heart Association (NYHA) functional class were eligible, but patients in NYHA class I could account for no more than 20% of the total number of patients undergoing randomization. Additional eligibility criteria were a modified Rankin score of 4 or less (on a scale of 0 to 6, with higher scores indicating more severe disability), and planned treatment with a beta-blocker, an angiotensin-converting–enzyme (ACE) inhibitor (or, if the side-effect profile with ACE inhibitors was unacceptable, with an angiotensin-receptor blocker), or hydralazine and nitrates. Patients were ineligible if they had a condition that conferred a high risk of cardiac embolism, such as atrial fibrillation, a mechanical cardiac valve, endocarditis, or an intracardiac mobile or pedunculated thrombus.

Eight to twelve week old CD57BL/6 mice were anesthetized with isoflurane inhalation, analgesia was performed by subcutaneous Buprenorphine injection. The ureter was separated from the surrounding tissues and two ligatures were placed about 5 mm apart in the upper two-thirds of the ureter of the left kidney to obtain reliable obstruction. On the same day after surgery and recovery, mice were treated with either vehicle buffer PBS, 5′-Azacytidine (10 mg/kg/day, Sigma, St. Louis, USA), low-dose, or high-dose Hydralazine (5 mg/kg/day or 50 mg/kg/day, respectively, Sigma, St. Louis, USA) given intraperitoneally every other day. Rats used included spontaneously hypertensive (SHR), Wistar Kyoto (WKY), and Wistar rats either sham-operated or subjected to a partial occlusion of the left renal artery (renovascular hypertensive, RVH)19 (link). Subgroups of SHRs were treated with losartan (20mg/kg/day, starting at 5 to 6-week-old, lasting for 7 weeks) or hydralazin (10mg/kg/day, starting at 8 to 9-week-old animals, lasting for 4 weeks) in drinking water20 (link),21 (link). Systolic blood pressure (BP) was measured using tail cuff method (coda 6; Kent Scientific Corporation, CT). All procedures were approved by the Georgia Regents University Institutional Animal Care and Use Committee Guidelines.

Human LECs cell line ((SRA01/04) immortalized with SV40 [73 (link)], a kind gift from late Dr. Venkat N. Reddy, Oakland University, Rochester, MI, USA) [73 (link)] were maintained routinely in our laboratory. Briefly, cells were cultured in a 100 mm tissue culture plate in Dulbecco’s Modified Eagle Medium (DMEM, Invitrogen, Waltham, MA, USA) supplemented with 15% heat-inactivated fetal bovine serum (FBS, Atlanta Biologicals, Atlanta, GA, USA), 100 µg/mL streptomycin, and 100 µg/mL penicillin in an incubator maintained at 37 °C with 5% CO2, as described previously [5 (link),74 (link)]. In the paper text, we will designate the immortalized LECs as SRA-hLECs. For the experimentation, SRA-hLECs were plated the day before treatment, and cells attaining 60–70% confluency were treated with nontoxic concentrations of hydralazine (Hyd).

MK2206, an AKT inhibitor, was purchased from Selleck (Shanghai, China). Intracellular reactive oxygen species (ROS) were detected via 2′,7′‐dichlorofluorescin diacetate (DCFDA) staining (Thermo Scientific, Waltham, MA, USA). Moreover, 4,4‐difluoro‐1,3,5,7,8‐pentamethyl‐4‐bora‐3a,4a‐diaza‐s‐indacene (BODIPY 493/503) was obtained from Thermo Scientific, and 4‐hydroxy‐trans‐2‐nonenal (4‐HNE)‐protein adducts in the indicated cells were detected through competitive enzyme‐linked immunosorbent assays (ELISAs) (Abcam, Cambridge, UK). Primary antibodies (Abs) against amphiregulin (AREG) (AB‐262‐NA) and basic fibroblast growth factor (bFGF) (AF‐233‐NA) were obtained from R&D Systems (Minneapolis, MN, USA).

Thirty-two compounds synthetized to target epigenetic pathways were studied (Figure 1). BIX-01294, apicidin, chaetocin, SGI-1027 and hydralazine were obtained from Sigma-Aldrich (St. Louis, MO, USA). TM2-115 was first synthetized at Imperial College London (M. Fuchter’s laboratory), United Kingdom and then obtained from Aobious (Gloucester, MA, USA). Trichostatin A, JIB-04, MG149 and C646 were obtained from Adooq Biosciences (Irvine, CA, USA). CB3717 was kindly provided by the Developmental Therapeutics Program, Division of Cancer Treatment and Diagnosis, National Cancer Institute (Frederick, MD, USA). SAHA, JAHA, PCI 34051, TC-H 106, MC 1568, tubastatin A, (+)-JQ-1, (+)-JD-1, SHH-878-1, AKE-040 [34 (link)] were from John Spencer’s laboratory and MZ1 was obtained from Biotechne, Tocris (Avonmouth, UK); https://www.tocris.com/products/mz-1_6154, accessed on 1 June 2021). Flv69 [35 (link)], Flv880 [36 (link)], MLo1302, MLo1401, MLo1406, MLo1502, MLo1507, MLo1508, MLo1509 [37 (link)] and NFlav2018 synthesized following our previously described procedures. Different batches of apicidin, BIX-01294 and TM2-115 were used during this work.
Artemisinin (ART) and atovaquone (ATQ) were purchased from TCI. Dihydroartemisinin (DHA) was provided by the WorldWide Antimalarial Resistance Network reference standards programme (WWARN, Bangkok, Thailand).
All compounds were prepared as stock solutions in 100% DMSO (Sigma-Aldrich).

Age, sex, body mass index (BMI), smoking status, ethnicity, previous exposure to relevant viral infections (Epstein-Barr virus (EBV), human cytomegalovirus (CMV), human herpesvirus 6 (HHV-6), human T lymphotropic virus type 1 (HTLV-1), hepatitis C virus (HCV), influenza A virus, and parvovirus B19), and previous prescriptions of selected medications (procainamide, hydralazine, quinidine, and isoniazid) were included as potential confounders. Previous studies found these variables to be associated with at least one of the outcome IMIDs and were thus adjusted for in the analysis [21 (link)–33 (link)].
Age was divided into the following bands: 18 to 29, 30 to 39, 40 to 49, 50 to 59, 60 to 69, and ≥ 70 years. Ethnicity was identified through SNOMED CT codes and was classified into the following groups: white, South Asian, black, mixed ethnicity and other. BMI was divided in accordance with the WHO classification: underweight (body mass index (BMI) < 18.5 kg/m2), normal weight (18.5–24.9 kg/m2), overweight (25–29.9 kg/m2) and obese (≥ 30 kg/m2) [34 ]. Smoking status was categorised as current smoker, ex-smoker and never smoked. A separate ‘data missing’ category was used where data were missing for ethnicity, smoking status, and BMI.
