The ED is staffed by board-certified or board-eligible emergency physicians 24 hours per day, 7 days a week who directly provide care and oversee care provided by residents, physician assistants, and nurse practitioners. ED patient volume in 2013 consisted of ≈104000 patient encounters. Cardiac testing modalities routinely available to study participants included exercise stress echocardiogram, dobutamine stress echocardiogram, coronary computed tomographic angiography, stress nuclear imaging, stress cardiac magnetic resonance imaging, or invasive coronary angiography. Serum troponin measurements were performed using the ADVIA Centaur platform TnI-Ultra™ assay (Siemens, Munich, Germany), which has a 99th percentile of the upper reference limit and 10% coefficient of variation at 0.04 mg/L.
