Placement of Composite Resin Restoration

After preparing the cavity, a metal matrix held by a Tofflemire retainer (S.S. White Dental Manufacturing Company, Philadelphia, PA, USA) was placed around the tooth. A self-etch adhesive (Clearfil SE Bond, Kuraray Okayama, Japan) was applied on the cavities with the tips of a disposable microbrush according to the manufacturer's instructions. Light curing was performed using a light-emitting diode light curing unit (Valo, Ultradent Products Inc., South Jordan, UT, USA) for 20 s. Increments of the composite resin were first applied (Clearfil Majesty Posterior, Kuraray Tokyo, Japan) to the gingival wall and then to the pulpal wall. The final increment was applied flush with the contour of the tooth. All the increments were light cured. Immediately after filling, the samples were finished with diamond burs at low speed with air-water spray and polished with a disc system (OptiDisc System, Kerr Corporation, Orange, CA, USA).

Composite Restoration: Surface Sealant Evaluation

After bonding procedures, Clearfil Majesty Posterior
(Kuraray, Okuyama Japan) composite material was
applied incrementally in two or three small horizontal
layers to cavities. Each layer was cured for 20 s with a
LED light curing unit (Elipar S10, 3M, ESPE, St Paul
MN, USA) having a minimal output of 1200 mW/
cm2. The light intensity output was monitored using a
dental radiometer (Hilux Ledmax Light Curing Meter,
Benlioğlu Dental Inc, Ankara, Turkey). Final countering
and finishing of the restorations were performed using
polishing points (PoGo, Dentsply) and flexible discs
and finishing strips (Sof Lex, 3M ESPE, St Paul MN,
USA). Following completion of the restorations, to one of
the teeth which was applied same bonding agent, was
covered of surface sealant material (Fortify Plus Surface
Sealant, Bisco, USA), other was not. Fortify Plus was
applied according to the manufacturer’s instructions to
a random selection of restorations. After the completion
of the restorative procedures, patients were contacted
and reexamined after 24 h and again after 7, 15, and
30 day to assess postoperative sensitivity. All cavities
were prepared and the restorations placed by the same
operator (S.A.G).

Adhesive System Effect on SFRC Restorations

Before the start of the study, the operative procedure was thoroughly discussed with the dentists. Three different adhesive systems were used randomly. Single-step self etch primer and bond (Vivapen, IvoclarVivadent, Schaan, Liechtenstein) and two-steps self etch primer and bond (Clearfil SE bond, Kuraray, Tokyo, Japan) and three-steps etching, primer and bond (Scotchbond multipurpose adhesive, 3M ESPE, USA). Bonding agents were used according to manufacturer's instructions. The SFRC (everX Posterior, GC Corporation, Tokyo, Japan) was placed and light-cured according to an incremental technique. Care was taken not to extend the SFRC base filling into the outer margins of the cavities. In all restorations the SFRC bulk base was covered with a surface layer of hybrid composite resin (1-2 mm). The composites used were: Estelite (Tokuyama, Japan), Clearfil Majesty Posterior (Kuraray, Japan), Z250 (3MESPE, USA), Z100 (3MESPE, USA), and Synergy (Coltène/Whaledent, Altstätten, Switzerland). Occlusal adjustment, finishing and polishing procedures were carried out predominantly at same visit.

Evaluation of Universal Bonding Agents

The teeth were randomly assigned to four groups (n=20) according to the bonding system used: All-Bond Universal (Bisco, Schaumburg, IL, USA); Clearfil Universal Bond (Kuraray Noritake Dental, Tokyo, Japan); Prime & Bond One Select (Dentsply DeTrey, Konstanz, Germany); and Single Bond Universal Adhesive (3M Deutschland, Neuss, Germany). The composition, batch numbers and manufacturers of the adhesive systems are listed in Table 1. Each group was divided into two subgroups according to the bonding technique performed: the self-etch technique or etch-and rinse technique. The adhesives were applied to the cavities according to the manufacturers' instructions and after restored with the selected manufacturers' recommended resin composites materials [Aelite All-Purpose Body (Bisco), Clearfil Majesty Posterior (Kuraray Noritake Dental), Ceram X Universal nano-ceramic (Dentsply DeTrey), and Filtek Z550 (3M ESPE)]. All procedures and curing times were performed according to the manufacturers' instructions (Table 1). All the teeth were then subjected to thermocycling for 2,500 cycles at a temperature of 5 and 55°C, with a dwell time of 10 s (THE-1100, SD Mechatronik, Feldkirchen-Westerham, Germany).
