Knee Cartilage Regeneration with hMSCs

The partial cartilage defect was introduced along the bilateral leg patellar groove of 8-week-old Sprague Dawley (SD) rats by biopsy punch (1.0-mm diameter, BP-10F, Kai Corporation, Tokyo, Japan) 200 µm × 9 mm2 in size (depth × area). After that, fibrin hydrogels (Beriplast-P Combi-set, CSL Behring, Marburg, Germany) with T1 hMSCs (group 1), T1 hMSCs treated with SF-NPs carrying SOX9 pDNA (group 2), T4 hMSCs (group 3), or T4 hMSCs treated with SF-NPs carrying SOX9 pDNA (T4, group 4) were embedded into the defect site in each leg. In each group, three rats (n=6) were sacrificed at 6 weeks after transplantation, and the legs were carefully excised for subsequent biological examinations. Regeneration of transplanted hMSCs was evaluated by RT-PCR and western blot.

Nonwoven PGA Mesh Tissue Repair Protocol

In this study we used a nonwoven PGA mesh (trade name: Neoveil, Gunze, Japan), fibrin glue (trade name: Beriplast P Combi-Set, CSL Behring, King of Prussia, PA, USA), sodium alginate powder (trade name: Alto, Kaigen, Japan) with a molecular weight ranging from 32,000 to 250,000, and calcium gluconate solution at a concentration of 8.5% (trade name: Calcicol, Nichi-Iko, Japan). The nonwoven PGA mesh was cut into square sheets 15 mm × 15 mm in size (PGA sheets) and sterilized with ethylene oxide for 22 hours, after which ethylene oxide gas was removed under conditions of decompression for one week. Fibrin glue was used according to the manual provided by the manufacturer in the form of a mixture of solutions A (fibrinogen and aprotinin) and B (thrombin and calcium chloride).

Cartilage Defect Repair via hMSC Transplantation

Eight-week-old Sprague Dawley (SD) rats were used in this study. After para-patellar arthrotomy, the patellar groove was exposed by laterally dislocating patella and partial cartilage defect along the groove by the biopsy punch (1.0mm diameter, BP-10F, Kai Corporation, Tokyo, Japan). To establish the mode, a defect (200 µm × 9 mm2; depth × area) was introduced into the knee (i.e., the site of patellar groove). Subsequently, prepared pellets of hMSCs (non-treated, sCGNP-treated, or mCGNP-treated) were transplanted at the defect sites in fibrin gel (Beriplast-P Combi-set, CSL Behring, Marburg, Germany). After the patella was relocated, the muscle and skin were sutured with a black silk 5-0 (SK526, Ailee Company Limited, Busan, Korea). At 1, 3, and 6 weeks after transplantation, three rats (two legs, n=6) from each group were sacrificed, the transplanted pellets were collected and the extent of repair was evaluated.

Fibrin-Encapsulated Rat Amnion-Derived MSCs

A total of 5 × 104 rat amnion-derived MSCs were suspended in 50 μl of fibrinogen solution (from Beriplast P Combi-set, CSL Behring). This suspension was then placed in a single well of a 24-well plate and 50 μl of thrombin was then rapidly added to the well and mixed using the pipette tip. This formed the FG-MSC complex, which added with 100 μl of growth medium (αMEM with 20% inactivated FBS containing l-glutamine, penicillin and streptomycin). For the Control group, 5 × 104 rat amnion-derived MSCs were plated on a 24-well plate with 200 μl growth medium. These were incubated at 37 °C and 5% CO2. At specific time points, cells were dissociated by adding 500 fibrin-degradation units of nattokinase dissolved in 100 μl of αMEM. This detached the MSCs from the well. Viability and number of collected MSCs were assessed using Countess Cell Counter (Invitrogen).

Lobar and Sublobar Resection Techniques

Lobar resection is a lobectomy (removal of a single lobe) or bilobectomy (removal of 2 adjacent lobes). Sublobar resection is wedge resection or segmentectomy. We routinely used the PGA sheet (Neoveil large size; 10.0 x 10.0 cm: Gunze, Kyoto, Japan) and 3 ml or 5 ml of fibrin glue (Bolheal: Teijin, Tokyo, Japan or Beriplast P Combi-Set: CSL Behring, Tokyo, Japan) to cover the dissected hilum when we performed a lobectomy, a bilobectomy and a segmentectomy, regardless of a lung parenchymal air leak. In the case of wedge resection, we covered them to the point of the air leak only when we confirmed the presence of an air leak.

Fetal Skin Transplantation in RA-Exposed Rats

Fetal intervention of RA-exposed Sprague-Dawley rats was performed at E20. Pregnant rats were anesthetized by isoflurane, and an abdominal midline incision was made to expose the uterine horns. The MMC defect was confirmed through the uterus under direct vision, and a small incision of the uterine wall and amniotic membranes was made directly above the defected site. 3D skin was transplanted into the area of defected skin. Beriplast P Combi-Set (CSL Behring, #87799) was used for tissue adhesion. The hysterectomy site was closed by purse-string suture with 6-0 silk (Natsume Seisakusho, #B10-60). The uterus was returned to the abdomen and the abdominal incision site was closed by running suture. The fetuses were harvested by cesarean section at E22. Pregnant rats were then euthanized by cervical dislocation under anesthesia with isoflurane. The transplantation site was dissected and processed for histological and immunohistochemistry analysis. The primary antibody list is provided in Table S2. Appropriate Alexia 488- or Alexa 594-conjugated secondary antibody was used with DAPI nuclear counterstain. The operation protocols were accepted by the Laboratory Animal Care and the Use Committee (A2015-003-C01).

Microsurgical Endonasal Transesphenoidal Approach

All the patients were treated by the same neurosurgeon (AC). The semi-sitting position was used, taking into account the concept of the spheno-seal point (2,3). A microscopic assisted endonasal trans-sphenoidal approach was performed (6).
During the nasal phase, nasal structures were preserved.
After detecting the intraoperative CSF leak, crural fat and fibrin glue (Beriplast ® P Combi set, CSL Behring, Montevideo, Uruguay) were placed.
The crural fat graft was collected using sterile dedicated instruments' set and was positioned in the space generated by the durotomy. The edges of the fat block were placed perfectly in-lay. Afterwards, the interface between the dura mater and the fat was sealed with fibrin glue. Finally, it was asked to the anesthesiologist to induce Valsalva's manoeuver to assess the efficacy of reconstruction.
No other hemostatic agents were used. During the reconstructive phase, the nasoseptal flap was not harvested. Lumbar drainage was not positioned. In the postoperative period, the patients stayed in bed for 48 hours with the head at 30 degrees. Pre and postoperative MRIs with detailed intraoperative images of six patients are shown in the Figures (Figure 123456).

Gelatin Glue and Fibrin Glue for Tissue Repair

Gelatin glue was prepared by mixing a 26% (wt/vol) aqueous solution of gelatin (alkaline-processed gelatin; mol wt 89,000; Nippi, Tokyo, Japan) and a 1% (wt/vol) glutaraldehyde solution (both sterile) at 38 C (Fig 1). Fibrin glue (Beriplast P Combi-Set; CSL Behring, Tokyo, Japan) was purchased from Waken Co Ltd (Osaka, Japan), and PGA sheets (Neoveil, 0.15 mm in thickness) were kindly supplied by GUNZE Ltd. Female Wistar ST rats (8 weeks old, weighing approximately 200 g) were purchased from Japan SLC Inc (Kyoto, Japan). Animal housing, care, and surgical procedures were performed in accordance with the institutional guidelines of the animal research committees of Nara Medical University.
