Formulation and Characterization of Cysteamine-Based Eye Drops

Cysteamine hydrochloride (cysteamine purity 97%) was obtained from Apollo Scientific (Stockport, UK) and 1-Heptanesulphone acid sodium salt was obtained from Sigma Aldrich (St. Louis, MO, USA). Cystamine dihydrochloride (Cystamine purity 97%) was obtained from Thermo Fisher Scientific (Geel, Belgium). Aquoral® eye drops (0.4% w/v % hyaluronic acid (HA), sodium chloride, sodium citrate, citric acid monohydrate, water for injectable preparations) were purchased from Esteve (Barcelona, Spain). Acetonitrile UHPLC-MS grade and acetic acid were purchased from VWR Chemicals (Radnor, PA, USA). Balanced Salt Solution (BSS®) was purchased from Alcon (Geneva, Switzerland). Ultrapure water from MilliQ, Merck Millipore (Madrid, Spain) was used. As a packaging material, COL Eye Drops System® made with polyvinyl chloride (PVC) and di-n-octyl phthalate/di(2-ethylhexyl) phthalate (DOP/DEHP), free from Biomed Device (Modena, Italy), were used as single-dose containers.

In Vitro Endothelial Cell Characterization

Human Endothelial Serum-Free Medium (SFM), 24 well tissue culture plates, ZO-1 antibodies and AlexaFluor 488 Goat anti-mouse IgG (H+L) antibodies were purchased from Fisher Bioblock Scientific (Vaulx-Milieu, France). Fibroblast Growth Factor (FGF-2), calcein, sodium dodecyl sulfate (SDS), propidium iodide (PI), trypsin/EDTA 1X and Hoechst 33342 were purchased from Sigma-Aldrich (Saint-Quentin Fallavier, France). Balanced Salt Solution (BSS) was purchased from Alcon (Rueil-Malmaison, France). Black carbon nanoparticles of 15-nanometer diameter (Black Pearls 470) were purchased from Cabot Corp and Business (Billerica, MA). Ti:Sa laser system was purchased from Thalès (Neuilly-sur-Seine, France). ANT-130 XYZ motorized stage and A3200 Software-Based Machine Controller were purchased from Aerotech (Pittsburgh, PA). The inverted fluorescence microscope was purchased from Olympus (Tokyo, Japan). FACSCalibur flow cytometer was purchased from Becton Dickinson (Franklin Lakes, NJ). Gentamycin was purchased from PAA Laboratories, (Pasching, Austria). Phalloidin was purchased from Cytoskeleton (Denver, CO).

Aqueous Humor Sampling for Protein Analysis

The AH samples of DME eyes were collected at baseline, and at 1, 3 and 12 months post first SMPL application, while the sampling of the non-diabetic control group was performed only once, as no changes of proteins’ concentration were expected through the time. All patients underwent the standard preoperative procedure: peri-ocular skin disinfection with povidone-iodine 10% (ESO-JOD, ECOLAB, Agrate Brianza, Italy), instillation of sterile lidocaine 4% (Alpha Intes, Napoli, Italy), irrigation of conjunctival sac with povidone-iodine 5% (Oftasteril, Alpha Intes) for two minutes and washing of the eye with Balanced Salt Solution (BSS; Alcon, Fort Worth, TX, USA). From the anterior chamber, AH (150–200 μL) was aspirated with an insulin syringe (31-gauge needle). AH samples were then collected in a single vial containing 10 μL of a cocktail of protease inhibitors (Pierce Biotechnology, Rock-ford, IL, USA) and quickly stored at −70 °C until analysis. The total protein content was quantified with a digital spectrophotometer (NanoDrop; Thermo Fisher Scientific Inc., Waltham, MA, USA) and protein concentrations were calculated by means of the linearized standard curve (BSA) and the A280 software. Thereafter, AH samples underwent sonication (VibraCell; Sonics, Newton, CT, USA) and centrifugation to collect clear supernatant (13000 rpm/7 min).
