Histopathological Evaluation of Liver Fibrosis

Paraffin embedded liver samples were cut in 5-µm thick sections and stained with haematoxylin-eosin (H&E), Masson’s trichrome, and Sirius red according to standard procedures. Liver necroinflammation was graded per the Ishak-modified Knodell Histological Activity Index (HAI), analyzing periportal and periseptal necrosis (score 0–4), confluent necrosis (score 0–6), focal inflammation (score 0–4), and portal inflammation (score 0–4). Fibrosis was assessed according to the scoring system proposed by Ishak et al. (no fibrosis 0 and cirrhosis 6) [51 (link)]. Analyses were performed in a blinded fashion and data presented as median and range. Quantitative analysis of collagen content was performed by computerized morphometric analysis on samples stained with Sirius red and digitally scanned by a high-resolution bright field and fluorescence slide scanner (Pannoramic MIDI BF/FL, 3DHISTECH Ltd., Budapest, Hungary). For this purpose, light microscope images (200X), excepting large centrilobular veins and large portal tracts were analyzed using Pannoramic Viewer 1.15.4 (3DHISTECH Ltd.).
