Biomarker Measurement Techniques

Biomarkers in Pediatric ICU Patients

We recorded the 24 h BLA values of the 90 patients at the time of admission into the PICU. BLA was measured with Vitros 250 full automation dipsticks analyzer manufactured by Johnson & Johnson (New Brunswick, NJ, USA). The venous blood was frozen for inspection. NCIS scale was used for the 90 patients in a retrospective manner. VIDAS BRAHMS PCT kit by bioMérieux (Marcy l'Etoile, France) was used for PCT. Enzyme-linked fluorescence assay (ELFA) technology was applied with the reference value of <0.5 ng/ml for newborns between 3 and 28 days. Boditech Med Inc. (Chuncheon, Korea) high-sensitivity C-Reactive Protein detection kit was used for high-sensitivity CRP. Immunofluorescence was applied with newborn reference value of ≤2 mg/l. Moreover, blood routine examination of the patients was carried out. Special Protein Analyzer manufactured by SEAC (Florence, Italy) was used for CRP inspection. Immune scatter turbidimetry method was used with a normal value of 0–0.8 mg/dl. The study started from the time when the patient was admitted into the Emergency ICU and ended at the time when the patient was discharged from the hospital or succumbed to the disease. Comparison was made on the difference of the BLA level, CRP and PRISM scores of the two groups.

Serum CRP and PCT Quantification Protocols

To compare gene expression classifier results to known biomarkers, we performed serum CRP and PCT testing. Quantitative measurement of human CRP by sandwich immunoassay with electrochemiluminescent detection was performed using 2 different platforms depending on the availability and country of location of serum samples. Some serum CRP measurements were performed at 1:1000 dilution, in duplicate, using Human CRP V-PLEX assay kit and QuickPlex SQ 120 Imager (Meso Scale Discovery, Rockville, MD; performed through the Duke Molecular Physiology Institute Biomarkers Core). Other serum CRP measurements were performed using the CRP XL assay kit (DiAgam, Ghislenghien, Belgium; performed through Durdans Laboratory, Sri Lanka).
Quantitative measurement of human PCT was also performed using 2 different platforms depending on the availability and location of serum samples. Some serum PCT measurements were conducted using the VIDAS BRAHMS PCT kit using the ELISA technique via the Mini-VIDAS platform (bioMérieux, Marcy-l’Étoile, France). Other serum PCT measurements were performed using the Exdia PCT (Precision Biosensor Inc., Daejeon, Korea; performed through Durdans Laboratory, Sri Lanka).
