Comparative Analysis of FC Measurement Kits

FC levels were determined with the same fecal sample using each of the following four kits, according to the protocol of manufacturer. Those included three standard kits; EliA® Calprotectin 2 (Thermo Fisher Scientific, Sweden), PhiCal® Calprotectin ELISA (Immundiagnostik, Germany)49 (link), and Buhlmann-E (BÜHLMANN fCAL® ELISA, Bühlmann, Switzerland), and also the new BÜHLMANN fCAL® turbo kit (Bühlmann, Switzerland)30 (link). All samples were measured in duplicate under the same pre-analytical conditions, with the mathematical mean used to reflect any imprecision for the patient samples. The characteristics of each kit are shown in Table S1. PhiCal assays were performed by SRL Inc. (Tokyo, Japan) as part of our previous prospective study, while the others included in the present study were performed by Nissui Pharmaceutical Co., Ltd. (Tokyo, Japan), after receiving the preserved fecal samples.

Fecal Biomarker Measurement Protocol

Parents were provided with a specific plastic screw-capped container and were instructed to collect a small amount of feces from the diapers immediately after defecation. The containers with the stool samples were kept in the fridge at home and then brought to the laboratory no later than 3 days after collection and were then stored at −20 °C until analysis. The protein extraction procedure of the samples, as a preliminary step of the analysis, was performed using the Fecal Sample Preparation Kit® (Roche Diagnostics, Rotkreuz, Switzerland) according to the manufacturer´s instructions.
The fCP and fEDN levels of stool samples were measured in duplicates, by the EliA Calprotectin 2 and ImmunoCAP EDN research assays (Thermo Fisher Scientific, Uppsala, Sweden), respectively. All samples pertaining to one child were analyzed in the same test run.

Quantification of Fecal Calprotectin

Faecal calprotectin (fCP) was determined using EliA Calprotectin 2 (Thermo Fisher Scientific, Uppsala, Sweden), using a Phadia 250 instrument according to the manufacturer’s instructions. Cut-off for positive results was 50 mg/kg according to the manufacturer’s instructions. Values below the lower limit of the measuring range (3.8 mg/kg) were assigned an arbitrary value of 1.9 mg/kg.

Fecal Calprotectin Quantification Protocol

EliATM Calprotectin 2 (Phadia AB, Uppsala, Sweden) kits were used to measure FC levels. Fecal samples were collected at home or in a hospital. When collected at home, patients were instructed to refrigerate their fecal samples (2–8°C). The fecal samples were stored in aliquots at −20°C or below until analysis using the fluorescence immunoassay method per manufacturer's instructions was performed. The EliA Calprotectin 2 kit measures FC levels in the range of 3.8 – ≥ 6,000 μg/g. As stated by the manufacturer, the cutoff level of FC, representing a positive value, was ≥ 50 μg/g. Based on previous studies.[13 ,15 (link)] the FC test results classified cases as follows: moderate to severe (>200 μg/g, >250 μg/g, or >300 μg/g), mild (100–200 μg/g, 100–250 μg/g, or 100–300 μg/g), and inactive (<100 μg/g) activity.
