Topical and Systemic Antifungal Treatment for Fungal Keratitis

Levofloxacin Eye Drop Protocol for Cataract Surgery

Ocular Infection Prevention Regimen

Rabbit Model of Ocular Injury

Healthy Belgian rabbits (3–6 months old, 2–2.5 kg) were divided into 4 groups: control group, OGI group, OGI with intravitreal Ranibizumab group, and OGI with intravitreal Conbercept group. Before OGI, general anesthesia was given via intraperitoneal injection of 3.5 mL/kg chloral hydrate (10%), followed by topical anesthesia with oxybuprocaine hydrochloride drops (Benoxil, 4 g/L, Santen, Japan). The OGI model was established by making a 5 mm penetrating wound circumferentially 6.0 mm behind the limbus at the supratemporal quadrant of the right eye, and the wound was sutured 0.5 hours later with aseptic technique. For anti-VEGF treatments, a single dose of 0.25 mg Ranibizumab (25 uL, Genentech, Inc., and Novartis International AG, Basel, Switzerland) or Conbercept (25 uL, Chengdu Kanghong Biotech Co., Ltd., Sichuan, China) was injected into the vitreous of the eyes after OGI at indicated time point (0.5 hours, 3 days, or 7 days), for each time point, n = 5 animals. Levofloxacin eye drops (0.5%, Santen, Japan) were used three times a day for three days before and after the eye injury and intravitreal injections. Two eyes in OGI group were excluded for endophthalmitis as we observed hypopyon or intravitreal empyema about 2 weeks after OGI. No injury was induced in the left eye.

Voriconazole and Amphotericin B Combination Therapy for Fungal Keratitis

In terms of medication, after 3–5 days, if the ulcer exhibited no healing tendency, the infiltration range exhibited no limiting tendency, and the infiltration depth was judged to be less than 1/2 of the corneal thickness through the slit-lamp microscope and anterior segment OCT results, patients who were unwilling to undergo keratoplasty or were not suitable for conjunctival flap covering were treated with the low-temperature plasma system for ablation.

Ocular Fibrosis Treatment Assay

In the vivo experiments, the assays include pentobarbital sodium (30 mg/kg) (Solarbio, Beijing, China), Benoxil (Santen, Japan), Tobramycin Dexamethasone Eye Ointment (Alcon, USA), 0.5% Levofloxacin Eye Drops (Santen, Japan), 4% Paraformaldehyde (Solarbio, Beijing, China), suture line (10-0 nylon; Alcon; USA), 8-0 Vicryl suture (Ethicon; USA), Normal Saline (Second Xiangya Hospital, Changsha, China), Mitomycin C (Sigma, USA), anti-Collagen I antibody (Abcam, UK), anti-α-SMA antibody (Abcam, UK), and anti-connective tissue growth factor (CTGF) antibody (Abcam, UK).

Fungal Keratitis Animal Model

Ten microliters of 2.4×106 CFU of the F. solanum spore suspension were injected into the corneal stroma with a microsyringe. Rat Model of Elevated Eye Pressure

In this study, the method of OHT induction was based mainly on the research of Mittag [22 (link)]. Rats were anesthetic by 1% sodium pentobarbital with the dosage of 0.4ml per 100 g weight. Incision of the conjunctiva exposed two dorsal episcleral veins located near the superior rectus and temporal episcleral vein near the lateral rectus. Then the veins were lift up and cauterized with an ophthalmic cautery carefully. Levofloxacin eye drops (Santen, Osaka, Japan) were used to diminish inflammation. After the conjunctival incision had healed (4-5 days after cauterization), a subconjunctival injection of 100μL of 5-fluorouracil (5-Fu, Haipu Pharmaceutical Co., Shanghai, China) with a concentration of 25mg/mL was also performed by a 29G needle on the EVC eyes. A week after the first injection of 5-Fu, a subconjunctival injection of 100μL of 5-Fu was performed again (only including Gw2, Gw4, and Gw8, because Gw1 had been executed at that time). Sham-operation eyes (SHO eyes) were sham-operated by receiving conjunctival incision and injection of 5-Fu at the same time with EVC eyes but without cauterization. All kinds of Blank-control eyes (BC-I eye, BC-II eye, and BC-III eye) received no treatment.

Corneal Alkali Burn Mouse Model

The CAB model was developed based on a previously described study, with midifications.1 (link) Briefly, mice were anesthetized using an intraperitoneal injection of 1.5% pentobarbital sodium (35–40 mg/kg). In addition, proparacaine hydrochloride eyedrop (Alkaline; Alcon, Belgium) was used for topical anesthesia. After exposing the eyeball, a filter paper (1.5 mm diameter), immersed in 2 µL 1N NaOH solution, was placed in the center of the right cornea for 50 seconds. After removing the filter paper, the eye was immediately rinsed with sterile saline until pH 7, and antibiotic ointment was applied. The left eyes were maintained as untreated internal controls. After the establishment of the animal model, the mice received levofloxacin eye drops (Cravit; Santen, China), thrice daily for 3 days.

3D Printed Ocular Bandage Lens Fabrication

Topical medications, including eye gel of deproteinized calf blood extract (20%, 5 g, Shenyang Xingqi Pharmaceutical Co., Ltd.), sodium hyaluronate eye drops (0.1%, 10 mL, Germany), levofloxacin eye drops (0.5%, Santen, Japan), gatifloxacin eye gel (0.3%, 5 g, Shenyang Xingqi Pharmaceutical Co., Ltd.), TobraDex eye drops (tobramycin 0.3% and dexamethasone 0.1%, 5 mL, Alcon, USA), TobraDex eye ointment (tobramycin 0.3% and dexamethasone 0.1%, 3.5 g, Alcon, USA), and Tacrolimus eye drops (0.1%, Santen, Japan) were used to promote epithelial repair, prevent infection, and alleviate inflammation, respectively. A 10-0 nylon suture (Alcon, USA) was used to secure the amniotic membrane.
