Automated Production and Quality Control of Ga-68 PSMA Tracer

68Gallium-HBED-CC was prepared using a GMP-grade 68Ge-68Ga-generator (GalliaPharm) in combination with an automated system (Modular-Lab Easy), cassettes and buffers as instructed by Eckert & Ziegler Eurotope (Berlin, Germany). 30μg PSMA-HBED-CC (ABX, Radeberg, Germany) was used per preparaton. Labelling quality control was performed by both instant thin layer chromatography and high performance liquid chromatography (Thermo Scientific Dionex UltiMate 3000), in combination with gamma detection. Images were acquired from skull vertex to the thighs using a TruePoint Biograph mCT40 scanner (Siemens, Erlangen, Germany), approximately 60 min after intravenous injection of 2 MBq/kg 68Ga-HBED-CC-Glu-NH-CO-NH-Lys(Ahx) or 2 MBq/kg FDG in case of FDG-PET/CT imaging. A low dose CT scan was performed using Care Dose 4D and Care kV, reference parameters: 40 mAs, 120 kV. Subsequently, PET was acquired according to the European Association of Nuclear Medicine (EANM) recommendations with the following parameters: PET with time-of-flight and point spread function (TrueX) reconstruction, 4 iterations, 21 subsets, with a filter of 7.5 mm full width at half maximum [36 (link)].

Standardized Radiolabeling and PET/MRI Imaging Protocol

A standard 68Ge/68Ga generator (Eckert and Ziegler) was used for elution of 68Ga prior to labelling with PSMA-HBED-CC (ABX GmbH, Radeberg, Germany)22 (link),23 (link),34 (link). After injection of 129.0 ± 26.2MBq of 68Ga-PSMA-HBED-CC, a low dose CT for attenuation correction (120 kVp, 30 mAs) and anatomical mapping was acquired within 89.0 ± 42.5 min immediately before the PET scan, using a Gemini TF 16 Astonish PET-CT scanner (Philips medical systems)35 (link). All 130 patients underwent a non-contrast-enhanced prostate multiparametric MRI at our institution at 3T (Magentom Skyra, Siemens Healthcare, Erlangen, Germany). Standard prostate MRI acquisition protocol included high resolution T2 weighted high resolution turbo spin echo sequences (T2 HR TSE, 25 slices, thickness 3 mm, gap 3 mm, TR 4040 ms, TE 116 ms, resolution: 0.47 × 0.47 mm), including DWI sequences (DWI, 25 slices, thickness 3 mm, gap 3 mm, TR 4800 ms, TE 58 ms, resolution 1.4 × 1.4 mm and b-factors 0/160/1200 s/mm2) with generation of an ADC map from b 0, b 160 and b 1200 images of the DWI. Native T1 high resolution isotropic volume sequences (T1 TSC, 49 slices thickness 5 mm, gap 6 mm, TE 11 ms, TR 800 ms, resolution 0.63 × 0.63 mm). No endorectal coils were used throughout all exams.

68Ga-PSMA-HBED-CC PET/CT Imaging

PET/CT imaging was performed 60.9 6 26.13 min after intravenous injection of 117.23 6 19.86 MBq (Table 2). A Gemini TF 16 PET/CT scanner (Philips) was used. The 3-dimensional acquisition mode was used for all PET scans. Axial, sagittal, and coronal slices were reconstructed (144 isotropic voxels 4 mm 3 each) using the standard reconstruction algorithm. Before the PET scan, a low-dose CT scan was obtained for anatomic mapping and attenuation correction (30 mAs, 120 kVp). Each bed position was acquired for 1.5 min, with a 50% overlap.
