In addition to the physical exam, participants were offered a CT scan of the chest. All CT scans were performed on an Aquilion One Vision Edition scanner (Toshiba Medical Systems, Otawara-shi, Japan) at the Department of Radiology, Rigshospitalet, Copenhagen, Denmark. The following settings were used for image acquisition: 120 kVp, automated exposure control (SD15) and reconstruction with filtered back projection and a soft tissue kernel (1mm slice thickness and 1mm increment). Scans were obtained during a deep inspiratory breath-hold and included the entire lungs.
