Individuals were consecutive referrals to specialty clinics in Ann Arbor, Michigan and Chicago, Illinois, and participants in research studies conducted through the University of North Carolina—Chapel Hill, University of Chicago, and University of Michigan. All participants provided informed consent and all procedures related to this project were approved by institutional review boards at the University of Chicago or University of Michigan.
